The Patient-Centered Medical Home is a model of care that puts patients at the forefront of care. It incorporates patient-standards emphasizing use of systematic, evidence-based, highly coordinated care that supports access, communication and a strong partnership between provider and patient.
The model improves quality of care and the patient experience while reducing socioeconomic disparities in health and healthcare costs. The agency’s clinical team works alongside teams of behavioral health providers, recovery professionals, pharmacists, social workers, and community advocates to care for each patient in an affirming and supportive environment.
We believe patient-centered care is an evidence-based solution. We utilize evidence-based care through real time data reviews to ensure our interventions match the needs of our communities.
One of the ways we apply this approach is through Project ECHO.

The Project ECHO (Extension for Community Healthcare Outcomes) was founded by Dr. Sanjeev Arora at the University of New Mexico on the philosophy of “All Teach, All Learn.” Participants of all backgrounds engage in a virtual community for learning and mentorship, to share critical expertise, provide peer support and guidance as well as discuss cases and updated care standards for the community served.
This collective understanding and engagement disseminates information of best practices across diverse disciplines to improve patient outcomes, create more health equity, and build strong healthcare systems locally, domestically and abroad. The ECHO model makes best practices and high-quality care available to everyone and we are leading the way in the HIV ECHO community.
Positive Impact Health Centers’ Project ECHO occurs the first and third Friday of each month. It starts with a 15 – 20 minutes discussion on current topics in HIV and infectious diseases. Then, our ECHO community presents real (anonymized) cases for discussion and recommendations. This continuous loop of learning is unique with a long-lasting impact for future generations of healthcare providers and their patients.
